2012 Registration Forms
All players completing in the Tri Town League must fill out the following 3 forms; 2012
Registration Form, Tri-Town Consent Form, and Medical Consent Form.
All other players including High School Players
must fill out two forms; 2012 Registration Form and Medical Consent Form.
Bring Completed forms and Check payable to
'LBA' to the registration dates posted on the home page, or to the Park and Rec office between 1/31/12 and 2/28/12.
